As the parent/legal guardian, I acknowledge the inherent risks of combat sports and agree to the Assumption of Risk, Liability Waiver, and Medical Release on behalf of the minor athlete.

I grant Unity Combat Series and Critical Sports and Entertainment permission to use photos and video of the athlete from official events for promotional purposes (optional).
